What to include in a resource request

You do not need to send medical records or other sensitive documents for an initial inquiry. A short description of the type of help you are seeking is usually enough for QOLCS to identify possible starting points.

  • Your name and preferred phone number or email address.
  • Your Florida county or Tampa Bay community so that regional resources can be considered.
  • Whether the request concerns senior services, veteran benefits, mobility, housing, employment, visual impairment, caregiving, wellness information, or another community need.
  • Any communication or accessibility preference, such as large-print information, email instead of phone, or having a caregiver participate.
  • A safe time to return your message during the Monday–Friday appointment window.

What happens next

  1. QOLCS reviews the request and identifies a relevant public agency, nonprofit, or educational resource when one is available.
  2. We may suggest questions to ask or information to gather before you contact that organization.
  3. The receiving organization makes its own eligibility, availability, and service decisions. A QOLCS referral is not an approval or guarantee.

Website privacy and safety

QOLCS does not request Social Security numbers, bank information, benefit passwords, full medical records, or payment-card details through ordinary email. Please do not send sensitive personal or health information in an initial message. Email is not an emergency or crisis channel.

This public website provides informational links and does not use a public intake form. Standard web-server security and operational logs may record technical information such as an IP address, browser type, requested page, and time of access. Third-party sites, including the secure Zeffy donation page, have their own privacy practices.
